How to Fill Out the Premium Entry Form

  • The Department # (e.g., Baked Goods, Crafts, etc.)
  • The Division # (subcategories within the department)
  • The Article # (specific item you're entering)
  • The Name of the Article (e.g., Apple Pie, Wooden Birdhouse)

Step 2: Complete the Entry Form

Fill in all parts of the form clearly:

  • Your name, phone number, and mailing address
  • Use a separate form for each department you enter
  • List all your entries in the table provided

Be sure to sign the bottom of the form—this confirms that your work is original, created by you, and hasn’t won first place at this fair in past years.

Step 3: Plan Your Drop-Off

Check the Premium Book for the entry drop-off date and time for each department you’re entering. Your items must be dropped off during those designated times.
